The flow
From booking to signature.
A form should be done before the client is in the chair, not while they’re sitting in it. Here is the path a consent form takes from the moment an appointment is made.
-
The booking triggers it
When a client books online, or joins the walk-in queue by scanning the QR code at the counter, the right form goes out automatically as an email or text link. Nobody has to remember to send it.
-
They fill it out at home
The form opens on their phone. Conditional logic hides the questions that don’t apply, and required fields can’t be skipped, so what comes back is complete the first time.
-
They sign and upload ID
An e-signature closes the form and a photo of their ID goes with it, so the age check is something you can produce on request months later.
-
It files itself
The signed form lands on that client’s profile next to their appointment history and past transactions. Pull it up from the dashboard in seconds, from any device, years later.
On the form
What shops usually capture.
Consent requirements differ by state and by the kind of work you do, so build the form your jurisdiction and your insurer ask for. These are the fields most shops end up with.
- Age and identity
- Date of birth, a signed statement that the client is of age, and an uploaded photo ID to back both up.
- Medical history
- Conditions, medications, and anything that changes how skin takes ink or how it heals afterwards.
- Allergies and reactions
- Latex, adhesives, pigments, and past reactions to tattooing or piercing work.
- Placement and design
- What is being done and where, agreed in writing before the machine comes out.
- Aftercare acknowledgment
- A record that the client received aftercare instructions and confirmed they understood them.
- Consent and release
- The signature itself, timestamped and stored against the client record.
The front desk
Where the paperwork comes from.
- Online booking
- Clients book 24/7, pay a deposit up front, and get reminded automatically.
- Shop queue
- Walk-ins and appointments share one line, with QR self check-in and a lobby display.
- Client management
- Contact details, appointment history, notes, and past transactions on one profile.
